Workforce Development of Frontline Trusted Messenger

The voice and role of health center community health workers (CHWs)/Promatoras, frontline staff, outreach & enrollment coordinators, and medical assistants within public health, the community, and in healthcare settings has always been as a trusted member with a close understanding of the community served. Recent reports have highlighted concerns regarding poor uptake of COVID-19 vaccines in groups and populations that have traditionally been underserved, including among healthcare workers in health centers. Educating communities on the COVID-19 vaccines’ safety and efficacy has emerged in the last year as one of the greatest challenges to address. We also recognize that vaccine misinformation is in circulation; therefore, this program has been developed to empower and support health center frontline staff, CHWs/promotores, outreach & enrollment coordinators, and medical assistants to readily provide factual truths in a balanced manner, as the trusted messengers in health centers.

Remote Patient Monitoring ECHO Project- August

Use of Remote Patient Monitoring (RPM) devices are defined as collecting and interpreting physiologic data that is digitally stored and/or transmitted by the patient and/or caregiver from outside the health center to the physician or qualified health care professional or team. Specific goals from using RPM can include improved patient knowledge, satisfaction and clinical outcomes as well as activating patients in self-management and adherence for their own health and care.

2022-2023 Quality Axis Learning Session II

The Quality Axis program supports health centers in their efforts to apply and institutionalize quality improvement strategies focused on access, workflows, and quality of care. All health centers are required to have QI programs due to regulatory requirements. The objective of this program is to improve the delivery of chronic disease care among participating health centers, with the 2022-2023 cohort focused on reducing the rate of poorly controlled diabetes (HbA1c >9%).
