As Federally Qualified Health Centers increasingly engage in population health work and collaborations with community partners, understanding the community in which they operate is necessary to reduce health disparities and improve health outcomes. Community Health Needs Assessments are a powerful means of identifying high priority needs, opportunities for intervention and investments. They are a means for identifying meaningful partnerships with other organizations needed to secure resources and to accomplish the mission of FQHCs.
For the first part of this two-part series, we welcomed Christina Holt from the Community Tool Box Services Center to present on developing and conducting community assessments, identifying assets and resources, and using these assessments to identify disparities and setting priorities. For the second part of this series, we will assign homework on the topic we learned about in Part 1 and then present the de-identified homework results so you can see how you compare to your peers, but also recommendations from TACHC staff for ensuring your Community Needs Assessments meet HRSA 330 requirements and support your Emergency Preparedness plans.
TACHC staff, from various departments, will be present to build on the information provided during Part 1 of this series.
