Thanks to the Human Resource and Services Administration’s (HRSA) support, TACHC is able to regionally host the National Committee for Quality Assurance’s (NCQA) Introduction to Patient Centered Medical Home (PCMH) and Advanced PCMH training programs. This training has been crafted to support community health centers in sustaining and achieving PCMH status. If both trainings are completed, this is also an opportunity for Primary Care Associations’ and community health centers’ staff to achieve PCMH Content Expert Certification (PCMH CCE) credential. Meals, materials and continuing education are included as part of the registration costs. Descriptions of each training are included below. Please also review the draft agendas for further information.

NCQA’s Introduction to PCMH Program: Foundational Concepts of the Medical Home (July 10-11)

This is a 1.5 day in-depth training on all of the 2017 NCQA PCMH standards and requirements for achieving PCMH recognition. This training is designed for any health care professional or administrator interested in understanding more thoroughly foundational aspects of NCQA’s PCMH. Participants will be able to hear directly from NCQA faculty on foundational concepts, best practices and relevant community health center specific examples to achieve PCMH recognition.

NCQA’s Advanced PCMH Program: Mastering the Medical Home Transformation (July 11-12)

This 1 day training is intended for individuals who have applied NCQA PCMH requirements in the practice settings. There are more hands-on activities to apply PCMH concepts, discuss autocredit, and analyze important aspects of a PCMH such as assessing social determinants of health, patient engagement and continuous quality improvement. This course is intended for individuals that have already attended the Introduction to PCMH Program, those who would like to pursue a PCMH CCE credential or any health care professional and administrator desiring a more in-depth training on NCQA’s PCMH 2017 standards and requirements.
