Unraveling PCMH Standards and Guidelines: PCMH Recognition Updates and Resources
Invitees: Community health center teams that include staff that lead and support PCMH efforts at the organization (i.e. IT, Leadership, QI/QA staff, PCMH Managers, PCMH Clinician Leads, etc.)
JSI will discuss the most current NCQA PCMH Standards and Guidelines. Health center staff will also have the opportunity to connect with fellow health center peers across the State to share best practices, support sustainability efforts, and align QI/QA initiatives at health centers with PCMH recognition. Updates will be provided on PCMH Standards and Guidelines since 2020 and distinction programs including Behavioral Health and Telehealth.
- Describe at least one specific update to NCQA’s PCMH Recognition Standards and Guidelines from the last year
- Describe at least one promising practice from a peer for achieving or sustaining PCMH recognition
- Recall at least one resource available through TACHC’s PCMH Resource Center
Please note: You will need to register for each call individually. Please do not forward your confirmation email or the Zoom meeting link to others as TACHC needs to gather attendance information for grant purposes.
