TACHC has developed web based learning courses specifically
for the Health Disparities Collaboratives. These courses are
available free of charge to centers currently participating
in a collaborative, or trying to implement spread. These courses
are currently being used by collaborative team members in
48 states.
Currently available:
- eLearning Overview
- Implementing the Chronic Care Model
- Diabetes
- Asthma
- Depression
- PECS
Courses include links to helpful websites and tools, and
video testimonials from providers who have already participated
in the collaborative. These courses are highly recommended
to complement the Learning Session experiences of team members,
to orient new team members to the collaborative, or to facilitate
spread of the collaborative activities. For more information,
