Preventive Maintenance Agreement

Henry Schein Preventive Maintenance Agreement

The following is a list of services typically performed as part of the Sullivan-Schein Dental (SSD)/TACHC Preventive Maintenance program:

Equipment Inventory and Program Customization:
· With the customer’s approval, SSD will conduct a complete inventory of dental equipment within the facility. This inventory will be provided to the customer and maintained by SSD.

Mechanical Room:
· Inspect compressor and change oil, drain tank, check filters.
· Inspect vacuum system, flush system and clean filters.

Sterilization Area:
· Inspect and clean autoclave/chemiclave to manufacturer specifications.
· Inspect and test all ultrasonic cleaners for proper operation.

Treatment Rooms:
· Inspect and clean patient light, check for drift and focus and adjust if necessary.
· Inspect control unit, hand pieces and W/W syringes for proper air pressure and water flow, check air and water filters.
· Inspect chair and unit assembly insuring all controls are operating properly and that the chair is operating within its full range of motion.

X-Ray:
· Inspect intra-oral x-ray equipment and insure it is operating properly. Check for drift and balance and adjust if necessary.
· Insect Panoramic x-ray equipment and insure it is operating properly.
· Inspect and clean x-ray processor.
· Check office calibration records.

Office Training:
· Train office personnel in the proper way to clean and maintain dental equipment.
· Specifically, focus on equipment that may have been neglected or maintained improperly in the past.

Other:
· Any other preventive maintenance service the customer requests.
· Preventive maintenance service does not include parts or solutions. Parts will be replaced only with customer’s approval.
· Labor rate may change over time and the length of each service call will vary based on the work requested.
· Labor and parts are invoiced after service is performed.
· Maintenance does not guarantee the elimination of equipment failures.
· Due to the age and condition of some equipment, preventive maintenance may not be available for that item.
