340Better Pharmacy Program

Retail Pharmacy Arrangements

It is possible to utilize the 340BetterSM program if your health center is contracting with a retail pharmacy for dispensing of 340B pharmaceuticals to health center patients. Below is a checklist of items that need to be completed when working with a contracted retail pharmacy.

  • A completed Self-Certification Form.
  • An original copy to Office of Pharmacy Affairs (OPA), with a copy to TACHC.
  • A copy of the Self-Certification Form can be found here (Requires Adobe Acrobat).

The health center must complete the 340Better Registration in order to utilize the 340Better pharmacy program. This 340Better Registration is to be completed by the health center using a contracted retail pharmacy.
