Reporting
Teams participating in the Health Disparities Collaborative are required to submit reports containing data on outcome measures and recent progress on a regular basis. Click here to view a Complete Guide to Reporting. (Word Document)

The components of a report
  • A Registry Summary Report is a report produced by PECS (Patient Electronic Care System) a software program used to record patient data for a center's population of focus and/or spread. This report may be customized to produce specific information about a population, or cover a broad range of data, depending on a health center's needs. Please note that PECS is currently only available to health centers participating in the Health Disparities Collaborative. To learn more, visit http://www.pecsusers.net/index.asp
  • An Excel Graphs Spreadsheet template aggregates outcome data on a monthly basis, and is customized per each health center's selected condition and measures. Graphs are produced showing overall trends and enabling centers to compare data from one month to the next in an easy, useful manner.
  • A Senior Leader Narrative is where teams record changes in their health system, list completed PDSA Cycles, challenges, successes, and assess their progress. Teams receive feedback from Cluster staff on this portion of the report.

Phase 1 teams are required to submit all three reporting components monthly: a PECS Registry Summary Report, Excel Graphs Spreadsheet, and Senior Leader Narrative. Phase 2 teams are required to submit a PECS Registry Summary Report(s) and an Excel Graphs Spreadsheet on a monthly basis. The HDNR Website (Health Disparities National Reporting)
HDNR is the web-based reporting site where Collaborative teams across the nation upload their monthly reports. The data is compiled and aggregated on a monthly basis to produce useful information on health trends and show the impact of Collaborative methods on chronic illnesses in patient populations. For purposes of uploading Registry Summary Reports to the HDNReporting site, teams with multiple Registry Summary Reports wanting to combine them into one Excel Workbook will find instructions on how to do so by clicking here.
