HDR (Health Disparities Reporting) is a web-based reporting site where Health Disparities Collaborative (HDC) teams upload their monthly reports. This reporting process is conducted to promote active learning about collaborative activities and outcomes at all levels. Monitoring the activity and results on a monthly/quarterly basis allows for timely assessment of progress with subsequent feedback, support, corrections and modifications, as needed. This process also helps to identify the successes, barriers, and trends, and provides a mechanism to share that information with the rest of the collaborative.

The aim of the HDR System is to provide timely data on the activities and outcomes of health centers participating in the Collaborative. The site stores monthly team data and allows for trend reporting. In addition, the website aggregates the data from all teams that have reported and automatically produces the monthly and quarterly summary reports.
